CMS Approved HOS-M 2023 Survey Vendors
CMS developed the HOS-M Vendor Program to establish standardized data collection to promote comparability of results across Programs of All Inclusive Care for the Elderly (PACE) organizations and Medicare Advantage Organizations (MAO). PACE organizations and MAOs select and contract with a CMS-approved HOS-M survey vendor to administer the HOS-M survey for PACE or Fully Integrated Dual Eligible (FIDE) Special Needs Plans (SNP) for frailty using a mixed mode (mail and telephone) protocol. To become a CMS-approved HOS-M survey vendor, an organization must demonstrate that it has the capability, experience, and personnel to collect and report accurate HOS-M survey results and successfully complete the annual HOS-M Survey Vendor training. The organizations listed below, in alphabetical order, have met the requirements to administer the 2023 HOS-M.
