2024 Health Outcomes Survey-Modified (HOS-M) Survey Vendors

The Centers for Medicare & Medicaid Services (CMS) developed the HOS-M Vendor Program to establish standardized data collection to promote comparability of results across Programs of All Inclusive Care for the Elderly (PACE) organizations and Medicare Advantage Organizations (MAO). PACE organizations and MAOs select and contract with a CMS-approved HOS-M survey vendor to administer the HOS-M survey for PACE or Fully Integrated Dual Eligible (FIDE) Special Needs Plans (SNP) for frailty using a mixed mode (mail and telephone) protocol. To become a CMS-approved HOS-M survey vendor, an organization must demonstrate that it has the capability, experience, and personnel to collect and report accurate HOS-M survey results. 

This table lists the survey vendors that were approved to administer the HOS-M in 2024. The 2025 approved survey vendors and updated contact information for the organizations will be posted here once the survey vendors successfully complete HOS-M survey vendor training in March 2025.
