Dissemination of HOS Results to MAOs

Each year a baseline report and a performance measurement report are disseminated to each Medicare Advantage Organization (MAO) participating in the Medicare HOS. Each participating MAO receives plan specific baseline and/or performance measurement reports, which present results for their MAO, the state total, and the HOS national total.

  • Beginning in 2013, each new MAO report file also includes a summary data file that contains contract-level responses to each HOS question, as well as demographic data related to the report.
  • Beginning in 2014, the HOS summary measures from each report that are used in the Medicare Part C Star Ratings are added to the associated summary data file (e.g., the HOS HEDISĀ® Effectiveness of Care measures are included in the Baseline Summary Data file and the HOS functional health measures are included in the Performance Measurement Summary Data file). Beneficiary level data files are made available to MAOs only after each cohort is complete.
  • Beginning in August 2014, with the advent of the regional Quality Innovation Network-Quality Improvement Organizations (QIN-QIOs), HOS state-level reports and data are no longer distributed to the QIOs.

Medicare HOS Baseline Results

After the administration of each baseline cohort, a cohort-specific baseline report is produced. The baseline reports present physical component summary and mental component summary scores. The scores are case mix adjusted using demographics, chronic medical conditions, and HOS study design variables. The baseline reports also provide MAO, state, and HOS national information on health status indicators and demographics. A sample baseline report (PDF, 1.2 MB) is available for download.


Medicare HOS Performance Measurement Results

After administration of each follow up cohort, a cohort-specific performance measurement report is produced. The performance measurement results describe change in health over time, which is characterized in terms of the direction and magnitude for a given respondent. These results account for demographic and health differences that may exist among members in the various MAOs. A sample performance measurement report (PDF, 1.0 MB) is available for download. The MAO Performance Measurement Contract List specifies the participating MAOs from the recently completed cohort.

After performance measurement reports have been distributed to the participating MAOs, beneficiary level performance measurement data are made available to the MAOs. In addition to the data files, each MAO is provided with a Data Users' Guide (DUG) that describes the Medicare HOS file specifications and the appropriate use of Medicare HOS performance measurement data. Availability of Reports and Data

Each summer, new releases of reports are made available to MAOs. For Cohorts 1-5 Baseline and Cohorts 1-2 Performance Measurement, hard copy reports were disseminated to MAOs via an overnight carrier. Effective Fall 2003, hard copy reports were no longer distributed to MAOs and all report distribution occurs electronically to participating MAOs through the CMS Health Plan Management System (HPMS). Baseline and Performance Measurement reports, as well as Star Ratings Validation and Aggregate Score Analysis tables, are available via HPMS through the Quality and Performance/HOS module. MAOs may contact their CMS Quality Point of Contact to obtain access to their HOS reports. An HPMS User ID is required to access HPMS. For assistance regarding HPMS access, please contact CMS via email at hpms_access@cms.hhs.gov. For earlier reports that were disseminated in hard copy format, electronic replacements can be ordered through the HOS Information and Technical Support email (hos@HCQIS.org). There is a modest replacement fee, which covers the cost of report reproduction. 

Each fall, new releases of data are made available to MAOs. New in 2016, the HOS data are sent by electronic Secure File Transfer to the designated MAO recipient (one per company) rather than by mailing CDs as in the past. Participating MAOs may obtain their data by contacting hos@HCQIS.org. The data recipient will receive an email from the hosdata@hsag.com mailbox containing instructions and a link to the Secure File Manager facility. Clicking on the link will allow for creation of an account and password establishment. The data will be available to download at that time.

Note: For Cohorts 1-16 Baseline and Cohorts 1-13 Performance Measurement, the state-level reports and data were also distributed to participating QIOs through the CMS QualityNet Application.

Information about previous distributions to QIOs is also available in the table. The grayed out cells indicate the data or reports that are not distributed to QIOs.
